Fixed Incorrect display of node permission (bold)

Hi

When a node has a individual permission set, it is displayed with bold letters in the node list. This works great.

But if I remove the individual permission (set all usergroups to inherit) the usergroup is display correct with normal letters but the node in the node list is still with bold letters.

Have noticed this too already.

And I think there should be a difference between a category (bold) and a "changed permissions forum" (bold)

It looks like this might be caused by some permission entries hanging around after deleting a user or user group. I've updated the query to ignore those and will fix the problem of them not being removed.
